Coverage Type:
- Benefits paid for a covered injury resulting directly from a Covered Accident
Covered Accident:
- Sudden, unforeseeable, external event causing Covered Injury or Covered Loss
- Excludes contribution from disease, sickness, or mental/bodily infirmity
- Not otherwise excluded under policy terms
Hospital:
- Licensed institution primarily providing medical care and treatment
- Managed under medical doctor supervision
- Provides 24-hour nursing services
- Excludes certain types of facilities
When Coverage Begins:
- Effective date: Later of the program’s effective date, eligibility, or the first of the month following receipt of enrollment form
- Active work requirement on effective date
When Coverage Ends:
- Ends when no longer eligible, group policy expires, or last paid premium date
- Continuation possible under certain circumstances
30 Day Right To Examine Certificate:
- Satisfaction guarantee; Certificate return within 30 days for a full premium refund
- Certificate void as if never issued

- Treatment within 30 days of the Covered Accident

- Diagnostic Exam:
